Step 1 — Full Record Extraction
AI MUST extract:
- All encounters and providers
- All timestamps
- All labs, imaging, medications
- All notes and communications
- All documentation patterns
YOU MUST CONFIRM:
- No missing time periods
- No missing providers
- No missing records
- Data matches source records
Step 2 — Clinical Filtering (What Matters)
AI filters for:
- Clinical decision points
- Changes in condition
- Escalation / failure to escalate
- Timing of care
- Standard-of-care expectations
YOU MUST CONFIRM:
- Only high-impact facts are included
- No irrelevant detail remains
- All escalation failures are captured
- All deterioration points are identified
Step 3 — Timeline Reconstruction
AI builds:
- Chronological timeline
- Event sequence
YOU MUST IDENTIFY:
- Inflection point (critical moment)
- Missed opportunity
- Delay in care
- Decision failure
Step 4 — Standard of Care
AI compares actions to general standards
YOU MUST CONFIRM:
- Was there a deviation?
- Was it clinically meaningful?
- Did it impact the outcome?
Step 5 — Causation (Critical)
AI builds:
- Temporal relationship
- Plausibility
- Alternative causes
YOU MUST DETERMINE:
- Did the failure cause the outcome?
- Is causation strong, moderate, or weak?
- What will opposing counsel argue?
AI does NOT decide causation. This is a human decision.
Step 6 — Litigation Intelligence
For every fact, ask:
- Does this support or weaken causation?
- Does this support or weaken breach?
- Does this affect damages?
- Will this survive cross-examination?
Final Check Before System Output
You must confirm:
- Timeline is accurate
- Inflection point is identified
- Breach is clearly defined (or ruled out)
- Causation is logically supported
- Only high-value facts remain
If any of these are unclear → STOP. Do not proceed to report generation.
